    Constructed as an UH-1B-BF by Bell at Fort Worth, Texas, USA.

    December 1964

    Taken on Strength/Charge with the United States Air Force with s/n 64-13917.

    9 September 1966

    Involved in an incident.
    After Hydraulic caution light began flashing, UH-1B 64-13917 precautionarily landed. Hydraluic failure was confirmed when right hose assembly was found finger tight at elbow fitting near gun mount.

    October 1966

    To Bell Helicopter Company, Fort Worth, TX.

    November 1966

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    March 1967

    Transferred to Post Supply, 6th Army, Fort Ord, CA.

    April 1967

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    June 1967

    Transferred to USARV Flight Detachment, South Vietnam.

    4 July 1967

    Damaged.
    While in target area over South Vietnam, UH-1B 64-13917 was attacking at 50 feet and UNK knots when it took 2 hits from from shrapnel in the cockpit. These hits damaged the main rotor system and the airframe. The helicopter continued flight and accomplished all mission objectives.

    August 1967

    To Bell Helicopter Company, Fort Worth, TX.

    November 1967

    Transferred to 355th Aviation Company, Phu Hiep, South Vietnam.

    23 December 1967

    Damaged.
    While in target area over South Vietnam, UH-1B 64-13917 was attacking at 10 feet and UNK knots. The helicopter was hit in the cockpit by two 7.62MM rounds, wounding both pilots. The helicopter continued flight and accomplished all mission objectives.

    28 December 1967

    Shot down.
    While in target area in South Vietnam this helicopter was attacking at 700 feet and 85 knots when it took 4 hits from enemy 7.62MM weapons. The helicopter was hit in the engine compartment causing a fire. Systems damaged were the engine, main rotor system, airframe and driveshaft. The helicopter made a forced landing down in trees. Aircraft was later recovered by any means other than its own power. Both mission and flight capability were terminated.

    Transferred to 605th Transportation Company, South Vietnam.

    March 1968

    To U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    November 1968

    Transferred to A Troop, 2-17th CAV, 101st CIB, 3rd Army, Fort Campbell, KY.

    April 1969

    Base of operations changed.
    Deployed to South Vietnam.

    June 1969

    Transferred to 142nd Transportation Company, South Vietnam.

    July 1969

    Transferred to 190th Assault Helicopter Company, South Vietnam.

    September 1970

    Transferred to 128th Assault Helicopter Company, South Vietnam.

    May 1971

    Transferred to 605th Transportation Company, South Vietnam.

    June 1971

    Transferred to U. S. Army Aviation Systems Command (AVSCOM), Saint. Louis, MO.

    July 1971

    To Bell Helicopter Company, Fort Worth, TX.

    January 1972

    Transferred to 84th Infantry Division, U. S. Army Reserves, Milwaukee, WI.

    September 1975

    Transferred to 88th Army Reserve Command (ARCOM), Fort Snelling, MN.

    To American Wings Air Museum, Anoka County Airport, Minneapolis-Saint Paul, MN.
