    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    1966

    Taken on Strength/Charge with the United States Air Force with s/n 65-9667.

    1966

    To A Company, 25th Aviation BAT., 25th INF., Cu Chi, South Vietnam.
    Flew combat missions in South Vietnam.

    8 January 1967

    Shot down.
    Helicopter took 1 hit from small arms to the equipment section while flying at an elevation of 10 feet and unknown speed. Damage to oil systems and engine. Made a forced Landing and was diverted after only completing some of the mission objectives. The helicopter was repaired in country.

    16 September 1967

    Damaged.
    Helicopter took 2 hits from small arms fire while orbiting at 1200 elevation and 70 knots. Helicopter was hit in the passenger compartment section. Damage done to the main rotor system. Made a precautionary landing and was diverted before accomplishing any mission objectives. The helicopter was repaired in country.

    4 September 1968

    Damaged.
    Aircraft hit by schrapnel while on ground at FireSupport Base Darby.

    Circa 1970

    Converted to an UH-1H.

    Transferred to North Carolina Army National Guard.

    From By November 1995 to After 2005

    To Carolinas Aviation Museum, Charlotte-Douglas International Airport, Charlotte, NC.
