    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    Circa March 1965

    Taken on Strength/Charge with the United States Army with s/n 64-13601.

    5 January 1966

    Damaged.
    During a troop infiltration mission, UH-1 64-13601 was damaged by an enemy rocket. The rocket damaged the oil systems, forcing UH-1 64-13601 down. After closer examination, the aircraft took off, fully flight capable. The aircraft was diverted or delayed after completing some mission objectives.

    To Bell Helicopter Company, Fort Worth, TX.
    Bailed for tests.

    November 1966

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    March 1967

    Transferred to Post Supply, 6th Army, Fort Ord, CA.

    April 1967

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.
    Deployed to Vietnam at this time.

    1 April 1967

    Damaged.
    This was a combat incident. This helicopter was repaired in theater. This was a Recon mission. While enroute this helicopter was Unknown at 1000 feet and 080 knots. Helicopter took 1 hit from small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size (7.62MM). The helicopter was hit in the Armament systems. The helicopter Continued Flight. The aircraft continued and accomplished all mission objectives.

    Transferred to U. S. Army, Republic of Vietnam (USARV) Flight Detachment.

    August 1967

    To Bell Helicopter Company, Fort Worth, TX.

    October 1967

    Transferred to 79th Transportation Company, Qui Nhon, South Vietnam.

    December 1967

    Transferred to U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), 4th ARMY, NAS Corpus Christi, TX.

    March 1968

    Transferred to 355th Aviation Company, 4th Army, Fort Sill, OK.

    April 1968

    To Bell Helicopter Company, Fort Worth, TX.

    May 1968

    Transferred to 355th Aviation Company, 4th Army, Fort Sill, OK.

    July 1968

    Transferred to OPM Sea NITEOPS, 1st Army, Fort Belvoir, VA.

    Transferred to Flight Training Center, Aviation School, Fort Stewart, GA.
