    Summarized NTSB narrative from report number CHI80FA054: There was one fatality. Incident occurred at 15:44 hours. The damage was substantial. This was a local flight from Toledo, OH. Landed wheels down in the water on touch down. The aircraft nosed over on touch down. The factors included; came to rest in water. Report remarks include: Passengers found in lower section of acft, no seat or seat belt installed, obs window found open. Incident occurred at or near Toledo, OH.
