    Transferred to 128th Assault Helicopter Company, Vietnam.

    From April 1969 to October 1969

    Returned to the United States for repair and overhaul.

    From October 1969 to April 1970

    Transferred to 118th Assault Helicopter Company, Vietnam.

    From April 1970 to May 1971

    To Corpus Christi, Texas, for a total overhaul.

    Transferred to Kentucky Army National Guard.

    Transferred to North Carolina Army National Guard.

    April 1977

    Converted.
    Converted by Bell.

    February 1994

    Struck off Strength/Charge from the United States Army.

    From Unknown to 1995

    Assigned to U. S. Army Aviation Museum, Fort Rucker, AL for storage.

    1995

    To Smithsonian National Air and Space Museum.

    Placed on display with Smithsonian, National Air and Space Museum, Steven F. Udvar-Hazy Center, Chantilly, Reston, VA.
