    Constructed as a XV-5A.

    1964

    Taken on Strength/Charge with the United States Army with s/n 62-4506.

    5 October 1966

    Crashed.
    Extensively damaged during trials as a rescue aircraft, when the test dummy was ingested into a wing fan. Pilot died of injuries.

    1967

    Taken on Strength/Charge with the National Aeronautics and Space Administration with s/n 705.

    Converted to a XV-5B.
    Repaired from crash and converted.

    24 June 1968

    First post-restoration Flight.

    25 June 1969

    To National Aeronautics and Space Administration, Moffett Field, CA with new c/r N705NA.
