    Constructed as a RF-4C-22-MC by at Saint Louis, Missouri, USA.

    26 August 1965

    First flight.

    Circa 22 October 1965

    Taken on Strength/Charge with the United States Air Force with s/n 64-1061.

    From By January 1968 to Circa 1969

    Transferred to 38th TRS, 26th TRG, Ramstein AB, West Germany.
    Operated with markings: RR
    Bill Kohler comments (15 May 2019): Was crew chief on this acft from Jan 1968 to approximately sometime in 1969 at the 38th TRS, Ramstein Germany.

    Damaged.
    On deployment to Sennacherib, West Germany, the aircraft crashed on landing. Aircraft was sent to McDonnell Douglas where it was repaired.

    January 1972

    Transferred to 30th TRS, 10th TRW, RAF Alconbury, England.
    Operated with markings: AS

    By January 1976

    Transferred to 179th TRS, 148th TRG, Minnesota ANG, Duluth IAP, MN.
    Operated with markings: MINNESOTA
    Served as the 148th TRG Commanders jet.
