    Constructed as a F-104A.

    Circa 1956

    Taken on Strength/Charge with the United States Air Force with s/n 56-0780.

    Taken on Strength/Charge with the Republic of China Air Force with s/n 56-0780.

    10 October 1964

    Involved in an incident.
    During the ROC independence day parade and air show it struct a radio tower with its left ext fuel tank, resulted in a fracture in air frame from the left intake to the rear of the left wing. Despite the incident, the pilot, Mjr. Chang Chia Jimmy was able to regain control of the plane and return to base.

    1965

    Repaired.
    Returned to Lockheed in California for repair.

    August 1965

    Returned to service.

    While in service was repaired with the installation of the tail from 56-779.
