    Constructed as a CH-47C(M).

    11 September 1968

    Taken on Strength/Charge with the United States Army with s/n 68-15814.

    From 1 December 1968 to 1 April 1971

    Transferred to 213th Assault Support Helicopter Company, Vietnam Black Cats.

    1972

    Redesignated as JCH-47C.
    Fitted with Helicopter Icing Spray System (HISS) and redesignated JCH-47C and used to produce ice clouds for test purposes.

    1984

    Transferred to US Army Aviation Engineering Flight Activity, Edwards Air Force Base, CA.

    The aft section was cut off and the rest (center fuselage) was sold to PlaneTags (MotoArt).
