    Constructed as a CH-47A.

    21 February 1967

    Taken on Strength/Charge with the United States Army with s/n 66-19058.

    Accumulated 3,151.3 aircraft hours.

    Remanufactured as a CH-47D.
    New construction number, M3017, assigned to the airframe.

    7 July 1983

    Taken on Strength/Charge with the United States Army with s/n 82-23766.

    19 November 1987

    Transferred to B Company, 159th Aviation Battalion, Fort Campbell, KY.

    Transferred to B Company, 7th Battalion, 101st Aviation Regiment, Fort Campbell, KY.

    By April 1998

    Transferred to E Company, 1st Battalion, 168th Aviation Regiment (ARNG), Pendleton, OR.

    Circa 2005

    Scrapped.
    The dynamic components, such as the engines, power train, rotor system and portions of the hydraulic system, etc., were removed and placed on a new build airframe MH-47G 05-03751 with remainder of airframe scrapped.
