    Constructed as an AH-1G-BF by Bell at Fort Worth, Texas, USA.

    April 1968

    Taken on Strength/Charge with the United States Army with s/n 67-15564.

    May 1968

    Transferred to 355th Aviation Company, Fort Sill, OK.

    From June 1968 to June 1970

    Transferred to 3rd Squadron, 17th Calvary Regiment, 101st Airborne Division, South Vietnam,.

    16 September 1968

    Damaged.
    Helicopter took 1 hits from: Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (7.62MM) Systems damaged were: INSTRUMENTS, STRUCTURE, PERSONNEL. Casualties = 01 WIA. The helicopter Continued Flight and was diverted prior to accomplishing any mission objectives.

    15 January 1969

    Damaged.
    This was a Combat incident. This helicopter was REPAIRED IN THEATER. This was a Recon mission for Armed Recon. While Enroute this helicopter was at Level Flight at 1200 feet and 110 knots. Helicopter took 1 hits from: Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (7.62MM), The helicopter was hit in the Bottom. Systems damaged were: STRUCTURE. The aircraft continued and accomplished all mission objectives.

    27 June 1970

    Damaged.
    This was a Combat incident. This helicopter was REPAIRED IN THEATER. This was a Recon mission for Armed Recon. While On Target this helicopter was Attacking at 0800 feet and 100 knots. In CambodiaHelicopter took 7 hits from: Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (12.7MM)Systems damaged were: TAIL ROTOR, STRUCTURE, ARMAMENT, TRANSMISSION, OIL SYSTEM. The helicopter made a Forced Landing. Aircraft later recovered by any means other than its own power. Both mission and flight capability were terminated.

    Transferred to 166th Transportation Company, South Vietnam.

    From September 1970 to February 1971

    Transferred to Army Aeronautical Depot Maintenance Center, NAS Corpus Christi, TX.

    From March 1971 to January 1972

    Transferred to A Battery, 4th Battalion, 77th Artillery Regiment, South Vietnam.

    March 1971

    Damaged.
    This was a Combat incident. This helicopter was REPAIRED IN THEATER, This was an Unknown mission While in Target Area this helicopter was Attacking at 3000 feet and 110 knots. In Laos. Helicopter took 2 hits from: Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (12.7MM). Systems damaged were: DRIVESHAFT, COMM SYSTEM, TAIL ROTOR. The aircraft continued and accomplished all mission objectives.

    Transferred to F Battery, 79th Aerial Rocket Artillery Battalion, 101st Airborne Division, South Vietnam.

    July 1972

    Transferred to 388th Transportation Company, South Vietnam.

    From August 1972 to December 1972

    Transferred to Army Aeronautical Depot Maintenance Center, NAS Corpus Christi, TX.

    January 1973

    Transferred to Army Aviation Systems Command, Saint. Louis, MO.
    Project loan.

    From February 1973 to June 1973

    Transferred to Army Aeronautical Depot Maintenance Center, NAS Corpus Christi, TX.

    From April 1973 to December 1975

    Transferred to 101st Airborne Division, Fort Campbell, KY.

    Circa 1976

    Converted to an AH-1S (Modified).

    By 1982

    Transferred to Company D, 7th Combat Aviation Battalion, Fort Ord, CA.

    1985

    Transferred to Company D, 307th Assault Helicopter Battalion, Fort Ord, CA.
    Unit redesignated.

    Circa September 1988

    Redesignated as AH-1S.

    Transferred to 1st Squadron 194th Cavalry (ARNG), Waterloo, IA.

    20 October 1997

    Transported by air. Delivered to Iowa Gold Star Military Museum, Camp Dodge NGB, Des Moines, IA.
    Aircraft was delivered from Waterloo, IA via sling under CH-47.
