    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    1966

    Taken on Strength/Charge with the United States Army with s/n 65-9643.

    October 1966

    Transferred to 174th Assault Helicopter Company, South Vietnam.

    May 1967

    Destroyed.
    Damaged by enemy fire while inserting an Eagle Flight north of Duc Pho. The aircraft was losing power and smoking badly. It was set down on the way back to Duc Pho. Later that day, before sunset, she was field stripped to deprive the Vietcong of the radios and equipment, then called in artillery to destroy aircraft.
