Flown as Elm 3 by Capt. Jack L. Francisco on a combat mission against the Yen Khoai barracks in North Vietnam. The aircraft was hit in the radome by flak. Flight lead was the 67th TFS c/o Lt. Col. Robinson Risner. |

Flown as Oak 2 by Capt. Goode on an Iron hand mission against a SAM site along Route Package 4 in North Vietnam. On this mission, Maj. Raymond James Merritt was flying as Oak 1 (F-105D 61-0189) and shot down by flak. Lt. Col. Robinson Risner, flying as Pepper 1 (F-105D 61-0127), was also shot down. Both of these 67th TFS pilots became POWs. Capt. Goode flew RESCAP for both pilots, then flew to Da Nang AB, South Vietnam, when he ran out of fuel. |

Damaged.
While flying on a combat mission, the aircraft received battle damage from enemy flak. The inboard pylons were damaged beyond repair and six feet of the leading edge flap was blown off. Aircraft returned to Takhli and was repaired. |

Damaged.
On a combat mission over North Vietnam, the aircraft recieved battle damage when two M-117 bombs struck the lower left and right side of the fuselage causing substantial damage. Aircraft returned to Takhli and was repaired. |

Assigned to 1st Lt. Bruce Cox, he named the aircraft Stephanie Allison after his daughter. Cox would fly this jet on more than 15 combat missions in early-mid 1970. |
